Can the AI still get something wrong?

Yes. A language model can paraphrase a sentence inaccurately. That is why the passage sits right beside it, so you check against the original rather than the summary. Statements without a source are marked or not shown at all.

Can an answer reveal documents someone may not open?

No. Permissions from your stores are checked on every query. What someone cannot open is neither searched nor quoted for them.

How do I know an answer rests on the current version?

Every citation names the document version. When two versions or two documents disagree, that goes into the contradiction list instead of being settled silently.

Can I trace later what an answer was based on?

Yes. The question, the passages used and the answer pass through the policy engine and are logged. You decide who may read the log.
